Integrative medicine encompasses all modalities of healing that have evidence. By using an integrative approach, we can help boost your wellness using a variety of modalities. Integrative medicine often uses conventional methods to provide a diagnosis. Treatments include complementary and alternative therapies that fall outside of the realm of conventional medicine.

Your treatment will be individually tailored after a thorough review of your medical history and will include lifestyle approaches, dietary supplements and botanicals, stress and mind-body techniques and possible referral to other complementary and whole systems based practitioners. Functional medicine treats you, the patient, not the disease. Functional medicine falls under the umbrella of integrative medicine and looks to uncover the root cause of illness to help you thrive not just survive. 

Cars run on different fuels ranging from diesel to gasoline to electricity. If you try to put gasoline into an electric car, not only will the car not run, you’ll likely cause some significant damage. Humans also vary and have many different requirements to ensure proper function. We have adapted to survive in particular environments and when our environments change faster than we can adapt, mismatch occurs.

Our genetics are like the hand of cards we’re dealt when we are born and how we play those cards is our epigenetics. Stress, exercise, sleep and diet will all affect whether or not our genetic predisposition becomes a disease. The way we live today and many of the foods we eat today would be unrecognizable by our great grandparents and three generations is not enough time for our bodies to adapt. This is likely in large part why 50 million Americans have auto-immune diseases (more than heart disease and cancer combined). Once you have one auto-immune disease, you are three times more likely to develop another auto-immune disease. The good news is through functional and ancestral medicine, we have tools to help you prevent or reverse auto-immune diseases.

By looking at each patient individually rather than treating the symptoms of a disease, we will help optimize function and reverse disease processes rather than settle for normal. Functional medicine uses a holistic approach to determine the root cause(s) of a disease or disorder. 

All aspects of your medical history are thoroughly reviewed to formulate your individualized treatment plan. Environmental and genetic factors may also be reviewed to determine underlying causes of disease and to gain deeper insight into your state of health. Treatment plans will address stress, diet, exercise and sleep to help you reverse or prevent illness. We may also use neurofeedback to optimize your brain waves. Because functional medicine focuses greatly on the personalization of medicine, it is considered by many the future model of healthcare and used in conjunction with precision medicine which targets therapies based on molecular diagnostics.

Where is Regenerative Medicine Typically Used?

Stellate Ganglion Block

A Stellate Ganglion Block is an outpatient procedure designed to address dysfunctional programming of the sympathetic portion of the autonomic nervous system (ANS). Typically, this programming results from exposure to extreme stress or trauma, often resulting in debilitating cognitive and psychological symptoms.

The sympathetic part of ANS controls the body’s “fight or flight” response, preparing the body for emergency situations. The sympathetic nervous system (SNS), also controls several other bodily systems including blood pressure, heartbeat, skin temperature, and the eyes’ response to light. Other functions controlled by the SNS include stress response, ability to focus, sexual function, and ability to relax.

When the SNS experiences overwhelm resulting from extreme stress or exposure to trauma, it becomes re-programmed to an unnaturally high set point. When this occurs, the body remains in a state of hyper-arousal, leading to inappropriate responses to stress. Individuals coping with dysfunctional autonomic programming may exhibit over-reactivity to stress, or conversely, emotional numbness. Because these symtoms stem from what is known as an “emotional injury,” they can often be corrected. The stellate ganglion block has shown consistently positive results in the treatment of the following conditions, among others:

  • Overreactive response to stress
  • Panic attacks
  • Post-traumatic stress disorder
  • Inability to relax
  • Reflex sympathetic dystrophy
  • Insomnia
  • Inability to focus & think clearly
  • Emotional numbness

Hydrodissection

Hyrodissection is essentially the release of a trapped nerve using fluid. Broken down, “hydro” means water, and “dissect” means to separate. Often, nerves become trapped as a result of tight muscles or connective tissue. When this occurs, circulation to the entrapped nerve decreases significantly, resulting in neuropathic symptoms such as numbness and tingling, radiating pain, “pins and needles,” weakness, and pain in the area around the nerve compression.

Lyftogt Perineural Injection

This treatment, first discovered and later significantly developed by Dr. John Lyftogt (pronounced “Liftoff”) consists of a series of small injections immediately under the skin targeting painful areas where the nerves are sensitive, with simple and natural substances. The substance Dr. Lyftogt uses is a buffered D5W (dextrose 5% in sterile water) with a neutral pH of 7.4.

Sphenopalatine Ganglion Block

The sphenopalatine ganglion is a grouping of nerves that is linked to the trigeminal nerve – the main culprit involved in headaches. If you apply numbing medications or local anesthetic to the SPG, it can help reduce pain related to headache or facial pain. A traditional sphenopalatine ganglion block involves application of an anesthetic solution either on a cotton swab or with a targeted injection through the face. More recently, these procedures have become less invasive, opting for an intranasal (up the nose) approach.

BR+ NAD Treatments


What is BR+ NAD Treatment? 

NAD has the ability to protect tissues, induce DNA repair and increase lifespan. Stress and inflammation lead to decreased levels of NAD. A nutrient-poor diet, and/or malabsorption, can also result in low NAD. Sub-optimal levels of NAD are often associated with health problems including addiction, degenerative diseases, anxiety, depression, PTSD, sleep disorders and autism. 

The fastest way to get your NAD levels up are with NAD IV treatments which directly raise your NAD levels. NAD BR+ is NAD Brain Restoration Therapy which is a nutrient-based IV therapy, pioneered by Dr. Richard Metsayer, that restores NAD. The NAD BR+ proprietary supplements are thought to be the best on the market by many. It’s important to know that if you have genetic mutations in your ability to methylate, the most common being the MTHFR gene, you will need to make sure you supplement with methyl donors such as creatine, trimethylglycine (betaine) or folate.

How does it work?

NAD is the rate limiting step, or “bottleneck”, for cell signaling. It’s also the control for many intracellular processes including DNA repair, apoptosis and telomere maintenance. DNA repair prevents damaged cells from causing further problems in our bodies such as cancer. Apoptosis is the body’s ability to identify damaged cells and kill them. Telomeres are like ends of our DNA. I like to think of them as the plastic ends on shoelaces; if they’re damaged, the DNA doesn’t replicate properly and the sequelae of aging ensues. (Stored in dropbox) Telomere length is one way to measure your biologic age, (how old your body is) vs chronologic age (how many years your body has lived).

Check out this diagram of the Krebs cycle (stored in dropbox). You’ll see some of the locations NAD+ is used in our metabolic systems. NAD is converted to NADH in metabolic pathways and the optimum NAD/NADH ratio is 700/1! In addition to apoptosis, DNA repair and telomere length, NAD also plays an important role in sirtuins.

Sirtuins are also getting a lot of buzz for their anti-aging effects. They regulate nuclear transcription and are intricately involved in the regulation of aging. A decreased NAD/NADH ratio suppresses the activity of Sirtuins. Oxygen indirectly catalyzes cellular reactions using NAD. Anything that causes decreased oxygen utilization results in a decreased ability of the mitochondria to recycle the NADH to NAD and the NAD/NADH ratio is decreased. Stellate Ganglion Block

A Stellate Ganglion Block is an outpatient procedure designed to address dysfunctional programming of the sympathetic portion of the autonomic nervous system (ANS). Typically, this programming results from exposure to extreme stress or trauma, often resulting in debilitating cognitive and psychological symptoms.

The sympathetic part of ANS controls the body’s “fight or flight” response, preparing the body for emergency situations. The sympathetic nervous system (SNS), also controls several other bodily systems including blood pressure, heartbeat, skin temperature, and the eyes’ response to light. Other functions controlled by the SNS include stress response, ability to focus, sexual function, and ability to relax.

When the SNS experiences overwhelm resulting from extreme stress or exposure to trauma, it becomes re-programmed to an unnaturally high set point. When this occurs, the body remains in a state of hyper-arousal, leading to inappropriate responses to stress. “This treatment, first discovered and later significantly developed by Dr. John Lyftogt (pronounced “Liftoff”) consists of a series of small injections immediately under the skin targeting painful areas where the nerves are sensitive, with simple and natural substances. The substance Dr. Lyftogt uses is a buffered D5W (dextrose 5% in sterile water) with a neutral pH of 7.4. Lyftogt Perineural Injection Treatment

The treatment is extremely safe; D5W is used as an IV solution in hospitals worldwide. Being mostly a subcutaneous treatment, and using only a half-inch needle, harm is minimized. There may be some bruising. No allergic reactions have yet been observed. Infection is extremely rare, calculated at about 1 in 300,000 injections. The only adverse reactions reported are temporary histamine induced welts at injection sites and extremely rare corn allergies. There are no reported issues of dependency. A Stellate Ganglion Block (or SGB) can act as a reset for the sympathetic nervous system.

The procedure itself involves injecting the area around the sympathetic ganglion at the level of C4 or C5 or C6 or a combination of levels with a long-acting local anesthetic (0.5% Ropivacaine) under ultrasound guidance.

The injections themselves only take a minute. Your practitioner will likely spend 15-20 minutes prior to the injections looking at neck anatomy with the ultrasound to find the best pathway to the proper location. You will lie on the table with your neck turned to one side for the procedure. If you feel too anxious to get an injection, we have various types of sedation which will allow you to relax for the procedure. After the injection, you will be watched for about an hour before going home. 

Stellate Ganglion Blocks

The injection can cause temporary Horner’s Syndrome which is characterized by a small pupil, a red eye, a droopy eyelid, a stuffy nostril and a change in sensation in the upper face. It lasts from 2-8 hours and will resolve on its own as the local anesthetic wears off.

SGBs have traditionally been used to treat upper limb pain conditions. Previously, SGBs were done without any sort of image guidance. Fluoroscopy was the gold standard for years. More recently, ultrasound has become the standard of care as it allows us to identify and avoid important structures in the area such as the carotid artery.

Antibodies are proteins that part of our immune system to fight infections in the body. There are several kinds of antibodies but the main ones that are important for infections are IgM and IgG. IgM is one of the first responder antibodies when your body is first infected. IgG plays a role later on in the course of the infection. Looking at both of these antibodies together, we can look at whether someone has had a recent infection or past exposure.

Testing for antibodies to COVID-19 might help identify people who could have been exposed or impacted by the virus (whether they knew it or not) and then later recovered. It is not entirely clear, at this point, if having an IgG antibody is an indicator of long term immunity to COVID-19; more research is needed.

Ketamine is a clear fluid that is commonly administered in a 40 minute IV drip for chronic pain or depression. Studies show that repeated ketamine drips may have a greater anti-depressive effect than a single ketamine drip. Ketamine can also be administered as an intramuscular injection, intranasally, subcutaneously, transdermally, transmucosally and orally. The reason we prefer injections is the oral bioavailability is only 17-29% so most of it is not absorbed. 

How does it work?

Let’s get into a little neuroscience. 

The central glutamatergic system, which is the main excitatory neurotransmitter in the nervous system, and NMDA receptors play a big role in pain and mood. There are three types of glutamate receptors: NMDA, AMPA and kainate receptors. When glutamate binds to these receptors it causes an influx of mainly sodium. This influx depolarizes the neuron and propagates an electrical action potential. 

When NMDA receptors are upregulated, pain signals are sent to the brain. Ketamine is a potent NMDA antagonist which decreases the pain signals. 

Ketamine blocks the NMDA receptor as you can see in the GIF but does not block the AMPA receptor. The AMPA receptor upregulation causes remodeling of synaptic connections and may explain why ketamine has effects beyond the actual infusion period. It also enhances anti-inflammatory effects in the central nervous system. Ketamine also has local anesthetic action and opioid receptor agonism. 

How long has ketamine been around? 

Named for the ketone and amine group in its chemical structure, Ketamine is a dissociative anesthetic that was first given to humans in 1964. It is used by veterinarians to sedate animals and has an expanding use in humans. In the early days, ketamine was found to be unsuitable for humans because it caused an emergent delirium when patients woke up and produced a sensory deprivation syndrome. Ketamine was approved by the FDA in 1970 and was used on injured soldiers during the Vietnam War. Platelet-rich plasma, commonly referred to as PRP, helps to stimulate the normal healing process in damaged or deteriorating tissues. It is derived from your own blood and contains approximately 5-10 times the platelet concentration of normal blood. Because platelets have tremendous healing power, PRP treatment can be used for a variety of musculoskeletal conditions.

Typical uses of PRP in musculoskeletal conditions include:

  • Chronic tendon problems
  • Muscle injuries
  • Ligament sprains
  • Tendon strain
  • Partial tears of ligaments or tendons

Typically, pain relief is experienced 3 to 4 weeks following the injection, with continued healing and improvement over a 3 to 6 month period.

How does PRP work?

In normal circumstances, the body’s first response to a soft tissue injury is to send platelets to the area to stimulate healing. With PRP injections, high concentrations of platelets are administered to the area, thereby accelerating the healing process.

PRP also contains concentrations of growth factors and cytokines which work to assist with cellular repair by improving cell growth, decreasing inflammation, aiding in immune system signaling, and activating metabolic pathways that assist with cell recovery. When administered in areas of injury or degeneration, platelets also attract the body’s autologous stem cells to the area to further stimulate repair and healing.

PRP injections are minimally invasive and, as such, do not require the extensive downtime associated with many other procedures. For this reason, they have become increasingly popular among athletes and active individuals.

What is a Sphenopalatine Ganglion Block?

The sphenopalatine ganglion is a grouping of nerves that is linked to the trigeminal nerve – the main culprit involved in headaches. A traditional sphenopalatine ganglion block involves application of an anesthetic solution either on a cotton swab or with a targeted injection through the face. How does RSWT Technology work?

How long will the pain relief last after treatment?

Most patients report some pain relief after the first treatment. However, the effects are most prominent after the second or third treatment. Your clinician will be able to help you through this process. Patients respond differently based on their condition.

Will I experience pain during treatment?

RSWT technology creates a mechanical stress that might cause mild discomfort during treatment. Depending on the desired depth of penetration, varying amounts of energy may be utilized to create the desired affect. Some soreness may be experienced post-treatment.

How many treatments are required on average?

The normal number of treatments for most conditions is 4 to 5. Certain conditions may require up to a maximum of 7 treatments. Four to ten days between treatments is recommended to allow the body to recover naturally.

Jon Vivolo, DO


Dr. Jon Vivolo DO was born and raised in Colorado and spent most of his youth here in Durango. He earned his undergraduate degree at Colorado State University in Health and Exercise Science with a focus on Sports Medicine and Nutrition in addition to Biomedical Sciences. During his undergraduate years he raced mountain bikes, winning the national championship and ultimately racing professionally. After an injury, he followed his passion for human physiology, sports performance, and community health optimization and completed his medical school training in Denver, Colorado at Rocky Vista University. In addition to his doctorate, he pursued an honors tract in rural and wilderness medicine prior to his emergency medicine residency training in Lexington, Kentucky. Following residency, he finished a fellowship in emergency ultrasonography at the University of Utah, focusing on ultrasound guided regional anesthesia and peri-arrest transesophageal echocardiogram for ECMO cannulation. He went on to practice in Salt Lake City at Intermountain Medical Center and helped train residents prior to moving back home to Durango. Through his practice in the emergency room, he saw firsthand the limitations of our current medical system in offering preventative, individualized, holistic medicine, which inspired him to complete an additional fellowship in genomics-based precision medicine. 

 

Jon has a passion for embracing our bodies' amazing innate ability to heal if given the proper support and environment.
